The following tools were used for the Advocates Mentoring Advocates Training Program. They may be downloaded and modified to suit your particular audience.
Sample Focus Group Moderator’s Guide
The moderator’s guide was used to facilitate the focus groups that were held early on. Focus group responses were used to help develop the training program.
Sample Workshop Evaluation Form
This was given to the advocates participating in the training program. They completed a separate evaluation for each workshop and their evaluations were used to help develop the Community Workshop.
Sample Overall Program Evaluation Form
This form was completed by the advocates who participated in the training. Their responses helped us to fine tune our program.
Pre- and Post-Community Workshop Knowledge Evaluation
This evaluation was given both at the beginning and at the end of the Community Workshop to assess the learning from the workshop. The correct answers are higlighted in yellow.
